The mission of the Chronic Disease Prevention Council Million Hearts Workgroup is to support and ensure the delivery of the Million Hearts® Initiative program; a nation-wide effort to prevent 1 million heart attacks and strokes by 2017. The Workgroup will partner with public and private entities to promote activities that address needs for education, environmental and systems changes in regards to cardiovascular disease prevention and cardiovascular health promotion.


The 2 key areas of focus are to improve clinical care and increase community activation.

Clinical care will emphasize the ABCS: Aspirin use as indicated; Blood pressure control; Cholesterol management; Smoking cessation. The New Mexico Million Hearts® workgroup has chosen to focus on Blood pressure control and Smoking cessation for many clinical and community activities.

Community activation will emphasize developing, coordinating, implementing and disseminating Million Hearts resources and education to health providers, health care workers and New Mexico communities.


The following goals will be accomplished by December 31, 2015:

  1. Community Activation and Clinical Care: provide training and supplies to at least 12 community and clinical organizations to conduct a Million Hearts activity appropriate for each setting. The purpose of the activity will be to help spread the word to the public about the Million Hearts Initiative, to share resources and educate the public about the importance of Self Measured Blood Pressure Monitoring (SMBP) plus additional support in controlling high blood pressure in clinical and/ or community settings.
  2. Clinical Care: Share information with at least 12 providers about existing Million Hearts guidelines to improve clinical quality care.
  3. Community Activation and Clinical Care: develop criteria to recognize partners for participating in the New Mexico Million Hearts Initiative.
